Forms are used to streamline communication about certain topics. For example, clients may use a form to formally request live monitoring or assessments, or the public may use a form to submit a noise complaint. Team members may also create custom forms to suit their needs. See Creating a Custom Form for more details on form creation and public and private forms.
To submit a form, navigate to the site of interest and click on the Form Submissions button. You will see a list of the form types you have access to. Click on any of them to see previous submissions or drafts of that form type. You will also see three new icons appear:
View Form
Open the form in a new tab. Here you can fill in a new copy of the selected form and submit it, or save it as a draft for later submission.
Download QR Code
Download a QR code image that you can embed in other media so others can easily access the form, e.g., in physical spaces.
Copy Link
Copy a shareable link that can be sent to others online.

Default Forms
Noise Complaint
Submit this form to make a formal complaint about noise produced from a site.
You will need to provide:
- Contact Information
- Full Name
- Address
- Phone Number
- Noise Details
- Whether the noise is a problem during the day or night
- Where the noise is most annoying (inside/outside)
- The perceived pitch of the noise (high/low)
- The nature of the noise (steady, intermittent, pulsating)
- Whether there is a noticeable tone produced
- Whether there is perceived vibration
- Any additional details about the noise
- Weather details at time of noise
- Temperature in Celsius
- Approximate wind speed in km/h
- Approximate wind direction
- Cloud coverage
- Precipitation
- Type of ground cover between your address and the site
- Any additional details about the weather
Noise Event Log
Submit this form to record a log about an instance or instances of noise produced by a site.
You will need to provide:
- Contact Information
- Full Name
- Address
- Noise Event (Per Instance)
- Date of the noise event
- Time of the noise event
- The nature of the noise (steady, intermittent, pulsating)
- Temperature in Celsius at the time of noise
- Approximate wind speed in km/h
- Approximate wind direction
- Type of ground cover between your address and the site
- Where the noise was heard (inside/outside)
